Whiplash is a soft tissue injury to the neck caused by a sudden, rapid movement, most commonly a rear-end motor vehicle collision, but also contact sport injuries, falls, or other impacts. Despite the mechanisms involved, whiplash injuries are frequently underestimated and undertreated. Research shows that a significant proportion of people who experience whiplash go on to develop persistent pain and disability, and that early, active management is one of the key factors predicting a better outcome.

What Happens in a Whiplash Injury?

  • Sprains of the facet joint capsules, the most common source of persistent whiplash pain.
  • Disc injury, particularly annular tears at the C5/6 and C6/7 levels.
  • Muscle and ligament strains, particularly the deep cervical flexors, upper trapezius, and levator scapulae.
  • Neurological involvement including nerve root irritation or, in severe cases, cord compromise (which will be referred to emergency services).

Symptoms

Whiplash symptoms may not fully develop for 24 to 72 hours after the incident. Common presentations include:

  • Neck pain and stiffness, often the primary complaint.
  • Headache, typically starting at the base of the skull and radiating forward.
  • Shoulder, upper back, and arm pain.
  • Jaw pain (TMJ involvement).
  • Tingling, numbness, or weakness in the arms (suggests nerve involvement).
  • Dizziness, visual disturbances, or tinnitus.
  • Cognitive changes, fatigue, and difficulty concentrating, particularly in higher-grade WAD.

Clinical Grading (WAD Classification)

  • WAD I: neck complaint with no physical signs.
  • WAD II: neck complaint with decreased range of motion and point tenderness.
  • WAD III: neck complaint with neurological signs including weakness, sensory changes, or reflex changes.
  • WAD IV: neck complaint with fracture or dislocation. Requires emergency care.

Treatment

Current clinical guidelines support early, active care for WAD rather than rest and a cervical collar. At Waterloo Chiropractic, treatment is evidence-based and adapted to your grade and presentation:

  • Gentle cervical mobilisation and manipulation to restore range of motion and reduce pain.
  • Soft tissue therapy for the cervical, upper thoracic, and shoulder girdle musculature.
  • Graduated active exercise to rebuild deep cervical flexor strength and proprioceptive control.
  • Pain neuroscience education to help reduce fear-avoidance behaviours that can prolong recovery.
  • Coordination with your GP or insurer if a CTP (Green Slip) claim is involved.
